Targeted Case Management is a program for people with MR/DD or individuals who are at risk for developmental disabilities or delays manifested before age 22.  The program assists recipients in gaining access to needed services described in the North Carolina State Medicaid Plan, as well as needed medical, social, educational, and other services

Obtaining Services

·         Individuals must complete a screening, triage, and referral (STR) to determine initial elibibility

·         All services must meet the medical necessity criteria and approval by a Medical Doctor

·         All services must have state prior approval

·         All services must be specified in the individual's person-centered-plan (PCP)

·         A PCP may be rejected by the state but individuals have the right to appeal the states decision

·         Individuals have  the right to choose their provider agency.
